This chapter is about kidney stones. Kidney stones have plagued humanity for millennia. Still, even Hippocrates, who was aware of the agony these stones can cause, warned against their treatment because he knew of the treatment risks. To this day, US medical students swear an oath which includes: “I will not cut the bladder stone”.

Things have changed. Open surgery, certainly risky procedures like during Hippocrates’ time are not done any more. They have given way to much less invasive, even gentle means to resolve stones of the kidneys, ureters, or the bladder.

Stones may be present in our systems, but we may not notice them at all. Stones only become noticeable when they become a problem. As long as the stay in the roomy insides of the kidney none notices their presence. Once they move into the narrow ureter and cause obstruction, however, they cause pressure build-up which we register as extreme pain.

The intense pain, any infection, or if the obstruction occurs in a single kidney are all causes to immediately proceed with stone removal.

This is possible with minimally invasive techniques: A tiny (1/16’’ diameter) camera on a flexible cable (an ureteroscope) is inserted into the tube connecting kidney and ureter. Once the stone is seen, a tiny laser fiber is inserted into the ureteroscope and laser light is sent through it. It is aimed at the stone, which causes the stone to break up into tiny pieces. Some of these are collected and sent for analysis; most of them are small enough that they are passed without problems during the day after surgery.

This type of surgery was introduced to the Pocatello are about 7 years ago. Hundreds of these procedures were successfully performed by Dr. Bjorn Sauerwein since 2007.
